Sloan Letter Folding Chart Description:

This folding card is ideal for vision screening. The letters range in size from 10/100 (20/200 equivalent) to 10/5 (20/10 equivalent). Lines 10/25 (20/50 equivalent) to 10/5 (20/10 equivalent) are in 3 groups to prevent memorization when testing with both eyes and each eye individually.

Specifications:
Has hole for wall hanging. Measures 18" x 18" and folds to 18" x 9".
